So here’s a look at how she managed to do this amazing feat.

Pain’s Invasion
Before talking about how Tsunade executed her mass healing, it’s important to know about the context of the situation first, which is the sudden and devastating invasion of Konoha by the leader of the Akatsuki, Pain Nagato.
Total Destruction
Pain brings about a level of destruction that the village has never seen before. In less than an hour,the six incarnations of Paindestroy over half of Konoha, kill countless civilians and shinobi alike, and interrogate lots of shinobi to find out Naruto’s whereabouts. Even elite shinobi like Kakashi and Guy succumb to his barrage of puzzling attacks.
Heroes and Saviors
Devastating as it is, it’s during these kinds of terrible moments that heroes are born. Fans know that Naruto will appear near the end of the invasion and eventually beat his misguided senior. And by the end of the day, Naruto has become the hero of Konoha.
However,it is actually Tsunadewho holds down the fort before the hero makes his late entrance. She is the one who has been protecting and healing as many injured citizens and shinobi all on her own. The Fifth Hokage casts a large-scale healing jutsu and becomes the savior of Konoha.

Reserve Seal: The Hundred Healings Mark
Tsunade’s Mitotic Regeneration begins with a sealing jutsu called Reverse Seal: The Hundred Healings Mark. This jutsu allows Tsunade to store her unused chakra into a special mark on her forehead.
When Tsunade activates Mitotic Regeneration, she unleashes the chakra stored within the seal, which allows her to forcibly stimulate the division of her cell. This means she can instantly heal any kind of injury on her body, no matter how severe it may be.

Temporal Invulnerability
I can regenerate organs and limbs. It’s not a recovery ability, it’s regeneration.
Being able to instantly heal any kind of injuries means as long as this jutsu is active, Tsunade is unkillable. She can even survive getting cleaved by Madara. When Pain starts to rampage all over Konoha, Tsunade quickly realizes that she can’t possibly save the people one by one. So she immediately decides to cast a large-scale healing jutsu so that she can save and protect them all at once.
This technique begins byactivating her Mitotic Regeneration Technique.After that, she summons Katsuyu, the mythical slug. Tsunade immediately synchronizes her chakra with Katsuyu so that she can channel her healing jutsu through the slug.

After that, Katsuyu divides her giant self into hundreds, if not thousands, of tiny Katsuyus. These little clones rush through the battlefield to find and latch on any wounded victims. Through this elaborate preparation, Tsunade is finally able to heal and protect almost every injured citizen and ninja of Konoha.
Burning Through the Fuel
In order to perform a miraculous feat such as instant regeneration, Mitotic Regeneration burns through a lot of chakra. Normally, it’s not an issue thanks to the massive amount of chakra stored with her Reverse Seal: The Hundred Healings Mark. At least that is the case when she only uses the technique on herself.
WhenTsunade begins to channel that chakra to hundreds of other people,however, it doesn’t take a long time for that well of chakra to dry up. In the span of a couple of hours, the large-scale healing jutsu has sucked up everything within Tsunade.
After the invasion has officially ended, Tsunade is unable to move a muscle in her body and passes out soon after. As if that’s not bad enough, her body also appears to undergo a rapid aging process, making her look like a frail 90-year-old woman.
